Use this subtask to select a new insurance election.

This subtask displays the short term disability plans that you can opt to elect.

Field Description
Select

Select this check box to choose this insurance plan as your new coverage.

Plan

This field displays the new insurance plan.

Coverage

This field displays your current level of coverage (for example, "Employee," "Employee + 1," "Family,").

Coverage Amount

This field displays the amount of coverage of the insurance.

<Frequency> (Negative indicates "Cash Back")

This field displays your current premium amount, based on the (current) pay frequency ("Monthly," "Semi-Monthly," "Bi-Weekly," or "Weekly").

Plan Information

This field displays the plan information if available. If the plan information is not available, “N/A” displays in the field.

Start Date This field displays the start date of the plan.
Evidence of Insurability

If applicable, this field displays if the coverage option requires Evidence of Insurability (EOI). Valid values are:

  • Not Required
  • Required
EOI Form Location If applicable, this field displays the location where the employee can download or find a copy of the EOI form to fill out.
