Use the fields and options to configure the Assign Beneficiaries to Benefit Plans screen.
Identification
Field | Description |

Employee |
Enter, or use to select, the ID number of the employee whose beneficiaries you want to assign to a benefit plan. Costpoint validates your entry against the data entered on the Manage Employee Information screen. This is a required field. |
Name |
This field displays the name of the employee. |
Plan Details
Field | Description |

Benefit Plan |
Enter, or click to select, the code for the benefit plan you want to assign to this beneficiary. Costpoint validates your entry against the Benefit Plans table. The description of the benefit plan displays in the field to the right. This is a required field. |
Effective Date |
Enter, or click to select, the date on which beneficiaries were assigned to this plan for this employee. The date must be in MM/DD/YYYY format. This field is required. |
Notary Date |
Enter, or click to select, the date on which this assignment was notarized. The date must be in MM/DD/YYYY format. |
Clauses
Field | Description |

Equal Shares or all to the survivors |
Select this check box if primary and/or contingent beneficiaries receive equal shares of the proceeds from the employee's life insurance plan. |
Unequal Shares or all to the survivors |
Select this check box if primary and/or contingent beneficiaries receive unequal shares of the proceeds from the employee's life insurance plan. |
Common Disaster Clause |
Select this check box if distribution of the proceeds of this employee's life insurance plan is governed by the Common Disaster Clause. |
Unborn Children Clause |
Select this check box if distribution of the proceeds of this employee's life insurance plan is affected by the Unborn Children Clause. |

Use this group box to define the type of beneficiaries for the employee. Click to create a new line.

Beneficiary Name |
This non-editable field displays the beneficiary's name, as specified on the Manage Employee Dependents/Beneficiaries screen. |
SSN |
This non-editable field displays the nine-digit Social Security Number of this beneficiary, as specified on the Manage Employee Dependents/Beneficiaries screen. |
Gender |
This field displays the code for the beneficiary's sex, depending on the option you selected in the Gender group box on the Manage Employee Dependents/Beneficiaries screen. |
Date of Birth |
This non-editable field displays the beneficiary's date of birth as specified on the Manage Employee Dependents/Beneficiaries screen. |
Relationship |
This non-editable field displays the beneficiary's relationship to this employee, as specified on the Manage Employee Dependents/Beneficiaries screen. |
Percent |
Enter the percent of shares this beneficiary receives in the event of a claim payout. This value must be between 1% and 100%.
This field is required. |
