Contents of the Assign Dependents to Benefit Plans Screen

Use the fields and options to configure the Assign Dependents to Benefit Plans screen.

Identification

Field Description
Employee

Enter, or use to select, the ID number of the employee whose dependents you want to assign to a benefit plan. Costpoint validates your entry against the data entered on the Manage Employee Information screen. The name of the employee displays in the adjacent, unlabeled field. This is a required field.

Benefit Plan

Enter, or use to select, the code for the benefit plan you want to assign to this dependent. Costpoint validates your entry against the data entered in Costpoint Benefits.

Before you can use this field, you must first:

  • Assign this benefit plan to this employee on the Manage Employee Benefit Elections screen.
  • Select the Dependents Eligible check box on the Coverage Options subtask of the Manage Benefit Plans screen for this benefit plan.

The description of the benefit plan displays in the adjacent field. This is a required field.

Table Window

Field Description
Coverage Option

The employee's coverage automatically displays in this field. However, the dependent can have a different coverage option if different coverages and rates apply.

If this coverage option uses a rate table, Costpoint uses the Dependent Rate per $1,000 to calculate the dependent portion of the premium that is added to the employee's premium. Otherwise, Costpoint calculates the dependent's premium and adds it to the employee's premium only if the coverage options are different. If you choose to override the default, the coverage option must be valid for this plan (as established on the Manage Benefit Plans screen). The coverage option cannot be NO CVG.  

If the dependent's coverage option differs from the employee's coverage option, Costpoint displays a warning message. However, you can update the dependent's coverage option.

If the dependents selected by the employee do not match the option selected in the Dependents Eligible field on the Coverage Options subtask of the Enter/Manage Benefit Plans screen, an error or a warning may display, depending on the Enrollment Rules Edit Method selected on the Configure Benefit Settings screen.

Dependent Name

Enter, or use to select, the ID number of the dependent for whom you want to override medical insurance enrollment.

Before you can use this field, you must first:

  • Link the dependent with this employee on the Enter/Manage Employee Dependents/Beneficiaries screen.
  • Select the Dependent or Both option in the Type group box on the Enter/Manage Employee Dependents/Beneficiaries screen.

In addition, a non-student dependent cannot be over the Dependent Maximum Age as stated on the Manage Benefit Plans screen. A student dependent cannot be over the Student Maximum Age as stated on the Eligibility Rules tab. The Dependents Eligible check box on the Coverage Options subtask must be selected, and if the Dependent Spouse or Child check box is selected, the Relationship, as specified on the Enter/Manage Employee Dependents/Beneficiaries screen, cannot be Other.

This is a required field.

SSN

This non-editable field displays the Social Security number of the dependent as entered on the Enter/Manage Employee Dependents/Beneficiaries screen.

Relationship

The relationship of this dependent to the employee, as entered on the Enter/Manage Employee Dependents/Beneficiaries screen, displays in this field.

Gender

The gender of this dependent, as entered on the Enter/Manage Employee Dependents/Beneficiaries screen, displays in this field.

Date of Birth

This non-editable field displays the date of birth of this dependent as it was entered on the Enter/Manage Employee Dependents/Beneficiaries screen.

Start Date

Enter the date, in MM/DD/YYYY format, on which this dependent's coverage begins, if the date differs from the employee's start date. The date must be within the employee's start/end date range as defined on the Enter/Manage Employee Benefit Elections screen. If you leave this field blank, Costpoint uses the employee's start date. This is an optional field.

End Date

Enter the date, in MM/DD/YYYY format, on which this dependent's coverage terminates, if the date differs from the employee's end date. The date must be within the employee's start/end date range as defined on the Enter/Manage Employee Benefit Elections screen. If you leave this field blank, Costpoint uses the employee's end date. This is an optional field.

Start/end date ranges for dependent coverages must be within the employee's start/end date range as defined on the Enter/Manage Employee Benefit Elections screen. If you selected only the Open Enrollment Period check box for this plan on the Enrollment/Coverage Rules tab of the Enter/Manage Benefit Plans screen, the system date must be within the enrollment date range (defined as enrollment date + grace period days). If you also selected the On Qualifying Life Event check box on that screen, you must either be within the open enrollment period, or the qualifying event date must be within the coverage option start/end date range from the Coverage Options subtask of the Enter/Manage Benefit Plans screen.

Primary Care Physician

Enter the name of the primary care physician in this field.

PCP#

Enter the primary care physician's ID number in this field.

Physician 2

Enter the name of an additional physician who is associated with the dependent/benefit plan.

Dentist

Enter the name of the dentist who is associated with the dependent/benefit plan.

Pharmacy

Enter the name of a pharmacy that is associated with the dependent/benefit plan.
