Customer Contact Info Tab
Use this tab to display information about the customer, and enter the email information related to the displayed information.

| Field | Description |

| Last Name | This field displays the last name of the customer contact associated with the RMA. If there are multiple contacts, this field will be blank. |
| First Name | This field displays the first name of the customer contact associated with the RMA. If there are multiple contacts, this field will be blank. |
| Phone | This field displays the phone number of the customer contact associated with the RMA. If there are multiple phone numbers, this field will be blank. |
| Fax | This field displays the fax number of the customer contact associated with the RMA. If there are multiple fax numbers, this field will be blank. |
| Email Address | Enter the email address of the customer contact associated with the RMA. |
| RMA Email Notification | Select this checkbox to send an email notification regarding the RMA information to the customer. |
